About Your ID Card

When you (or a family member) receives one of the implantable device types listed below, you will receive a device ID card. Your hospital will submit a device registration form to us and give you a temporary ID card. We will then mail you a permanent card within 30 to 90 days.

Verify your information

When you receive your permanent plastic ID card, check the information on it carefully.

If you do not receive your card or your card has any incorrect information on it, please contact us.

Carry your ID card

Always carry your ID card with you.

Your card shows the contact info of your physician or clinic (which those assisting you during an emergency would need to know). It also lists out MRI safety information.

Protecting your privacy is important to us

The United States Food and Drug Administration (FDA) requires we maintain up-to-date patient records. By submitting your information, you agree Abbott may keep a record of your information and use it to respond to your requests.

All information you provide is secure and confidential, and only authorized Abbott personnel have access to it. We won't release or publish any of your information without your approval. Your physician and regulatory agencies, such as the FDA, have the ability to request this information. Review our privacy policy.
